Yeah, Dean hasn't been shown getting worst.
Just like every time someone references the horrors of him being a demon I kinda fill in the, you know - "I can't see you become a demon again, Dean. Your singing was horrendous".

But I d obelieve that Sam is super worried, just cause he's seeing Dean's nerves fraying and - like it's almost unbearable for me to watch, as well - Dean without hope. Its so heavy. dark.
